I hereby apply to the Board of Directors for membership of my immediate family in the Azalea Tennis and Swimming Club of Dothan, Alabama. By submitting this application, I understand that I will pay a non-refundable membership initiation fee of $600 (one time fee), annual dues of $475 and capital improvement as deemed necessary by the Board. Membership year begins May 1st and annual dues should be received by June 1st each year to maintain membership status. Applications will be received and responded to throughout the calendar year; dues prorated accordingly.
